Forms

In preparation for getting a new job, your student employment file must be complete.  Before you can begin work, you must complete and submit all of the following forms to the Financial Aid Office.

Work Study Confidentiality Agreement

Automatic Work Study Direct Deposit Authorization

W4 Form

I-9 Form

Note:  The I-9 Form is a federal document which requires original forms of identification and employment eligibility documents must be presented in person.  Refer to the I-9 Form list of Acceptable Documents.  The I-9 Form must be completed during Fall 2008 Registration.
